BPC-157 vs TB-500: Which Is Better for Healing?

Overview
BPC-157 and TB-500 are the two most popular peptides in the healing and recovery space. Both are studied for their ability to accelerate tissue repair, but they work through entirely different mechanisms and have different strengths. Understanding these differences is key to choosing the right one — or deciding whether to use both together.
How BPC-157 Works
BPC-157 (Body Protection Compound-157) is a 15-amino-acid peptide derived from a protective protein found in human gastric juice. It promotes healing primarily by upregulating growth factor expression, stimulating angiogenesis (new blood vessel formation), and modulating the nitric oxide system. BPC-157 is particularly well-studied for gut healing, tendon repair, and ligament injuries. It also appears to have protective effects on the liver and brain. Most of the research is from animal studies — there are no completed human clinical trials as of 2026, though several are underway.
How TB-500 Works
TB-500 is a synthetic version of Thymosin Beta-4, a 43-amino-acid peptide naturally produced by the thymus gland. It promotes healing through a different pathway: TB-500 upregulates actin, a cell-building protein critical for cell migration and proliferation. This allows cells to move to injury sites faster and begin repair. TB-500 also reduces inflammation and has systemic effects — unlike BPC-157, which tends to work best near the administration site, TB-500 distributes throughout the body regardless of where it is injected.
Comparing Their Strengths
BPC-157 excels at localized healing. It is the stronger choice for gut-related issues (leaky gut, IBS, ulcers), tendon and ligament injuries, and situations where you can inject near the injury site. It also has neuroprotective properties and may help with dopamine system recovery.
TB-500 is better for systemic healing. It works well for muscle tears, cardiac tissue repair, and widespread inflammation. Because it distributes throughout the body, injection location matters less. TB-500 is often preferred for general recovery and chronic inflammatory conditions.
For acute, localized injuries — BPC-157. For systemic recovery and muscle repair — TB-500. For serious injuries or accelerated healing — many protocols combine both.
Dosage Comparison
BPC-157 is typically dosed at 200-800 mcg per day, administered subcutaneously near the injury site or orally for gut conditions. It is often cycled 4-6 weeks on, 2 weeks off.
TB-500 uses a loading protocol: 2-5 mg subcutaneously twice weekly for 4-6 weeks, then a maintenance dose of 2-5 mg every 2 weeks. The higher dosing reflects its lower potency per milligram compared to BPC-157.
When stacking both: BPC-157 500 mcg + TB-500 2.5 mg subcutaneously two or three times weekly for 4-8 weeks is a common protocol.
Side Effects and Safety
BPC-157 side effects are generally mild: nausea, dizziness, headache, and injection site irritation. The main theoretical concern is that it promotes angiogenesis, which could theoretically accelerate growth of existing tumors.
TB-500 has a similar side effect profile: headaches, nausea, fatigue, and injection site irritation. It shares the same angiogenesis concern. TB-500 may also promote cell migration in ways that could be problematic if cancer is present.
Neither peptide has completed human clinical trials, so long-term safety data is not available for either compound.
The Verdict
There is no single "better" peptide — it depends on the injury. BPC-157 is the go-to for gut healing and localized tendon/ligament repair. TB-500 is preferred for systemic recovery, muscle injuries, and situations where you need whole-body healing. The combination of both is widely used for serious injuries and is supported by the fact that they work through complementary mechanisms.
